The level design in Assassin's Creed 2: Discovery.

The challenge missions are really fucking frustrating! You may not be able to tell just from watching it on Youtube, but believe me that games really fucking hard.

Thankfully it's only hard if you're trying to get 100% completion and I still recoment it to anyone with a DS and a love for side scrollers.

Even with 100% chameleon sometimes the guards would bug out and detect me while I was killing people. It was hilarious watching them swarm the city streets looking for me while I was killing them.
Technically that's not a bug; guards get some kind of psychic spidey-sense whenever you commit an actual crime around them, but that AI is overridden if you choose "resist arrest"---they try to kill you, but they can't find you.

being able to save everywhere, in games like Fallout 3 (repeating speech challenges or computer hackings) Oblivion (repeating auto lock-pick attempts) And Mass Effect (battles and conversations)

Is this bad design, or am I that good?

Dead Ringer in TF2.

Sorry, a kill is a bloody kill, can't freaking stand this "is he even dead" bullshit.

I'd be completely fine with the weapon if they JUST went invisible on a single hit but dropping the fake corpse... that just makes everyone second guess every spy kill.

Oh yeah, and how in that invisible state there is a 90% damage reduction, that is bullshit. Spy effectively becomes the most powerful enemy in the game, effective 1250hp, over twice that of a buffed Heavy. A pyro takes forever to kill one and normally enough time to escape to the enemy front line.

OVERPOWERED:
From Demon's Souls we have the Mirdan Hammer which does very high damage when upgraded, attacks very quickly, has a very long range, and can destroy blockers. Though Northern Regalia has quite a bit higher damage, the Hammer is much faster and longer range. Doesn't help that 1 setup gives it HP regen. Combine that, with the HP regen shield and the regen ring to have a very cheap setup.

Gundam in Gundam: Crossfire is ubertastic. Though late in the game very expensive, tis neigh-indestructible. I have it maxed and 2 others maxed for my allies and nothing stands a chance. Even the final level on the hardest difficulty was a cake-walk. Normally your allies last half as long as a doughnut in a police station, but even they can't die when piloting Gundam.

Urban Cohorts in Rome: Total War. Fairly quick, they have a very powerful ranged attack (though only 2 volleys), very high armor, and will defeat ANY other infantry head-on (even if you run headlong into the speartips of a Spartan phalanx assuming you hit the Spartans with the javelins).They are also a LOT cheaper than the only infantry that can stand up to them. Praetorian Cavalry also get mentioned for being able to best even Cataphracts (normally the best heavy horsemen in the game). All online Roman players LOVE both Praetorian Cav and Urbs.

Snake in SSBB has quick attacks, oddly large range, and high damage and knockback. His basic combo can often KO middle weights at 60%. He is mostly unmatched in ranged warfare and is a beast in melee. He can move fairly quickly and has great recovery. MetaKnight is said to be the most unbalanced, but he never fairs well in my Smash group. Another special mention to Pikachu's Down special, the thunder of uber cheapness and G&W's hammer.

If you thought MW2's noobtube was overpowered, just look at what we can expect from Brink's (Bethesda's upcoming shooter) noobtube:
I mean, just look at 1:17; aim in general direction, floomp, triple kill. I mean come on. If they're going to release the game like that, we'll see a LOT of grenade spamming.

I heard this story about 2 paladins guarding the last boss in Alterac Valley against a full 40 man Horde raid. At that point I was like "Heej now..."

That's why I love my clan's servers. We have this mod that lets corpses disappear, with some animation out HL2 (you know when you shoot those energy balls at a Combine, or the alt fire of the Pulse Rifle). A Dead Ringer Spy's corpse does not do this. We have the mod to reduce framerate lag, but it's a funny little extra. And even without that mod, you can pay attention to gibbing. If a Spy doesn't 'splode after being shot by a rocket; Dead Ringer.

The ultimate sword in Disgaea: HoD, its called something the like Youshimin.

It has the same range as a gun, by the time you have it the game is cake walk, but its going to take 50+ of grinding to get it...

Generally randomly fired noob tubes in both Modern Warfare 2 and Bad Company 2. Those things irratate me more than snipers.
