Nemesis, from Resident Evil 3. Is not that he is scary to look, but he is indefeatable, he can pop on ANY screen when you are in the streets, and when he appears usually means "drop everything and run for your life without looking back".

I actually have a few.

Necromorphs from Dead Space/2: They will not stop until they are nothing but a stump, that my friends is a true zombie. Specifically the Regenerator...that's just freaky because you know that you can beat it and it is right behind you.

Water Lurkers from Amnesia The Dark Descent: Any enemy from this game could count but these ones particularly, you hear their footsteps and maybe see the water rising and your blood starts pumping like never before. These things and invisible and in the demo I was on a box watching the footsteps...this thing...this Cosmic Horror was right in front of me and I wouldn't even know it. The worst part was when you had to get in the water and open the gate.

Librarians from Metro 2033: Thankfully you can see these ones, but unfortunately they are damn near invincible and will do A LOT of damage in one hit. The specific point at which it gets adrenalin-inducing is when you're running through the library with Librarians popping out from everywhere, either that or when I had to try and sneak passed the sleeping ones.

Dr. Salvador from Resident Evil 4: I'm not gonna lie, up until the castle Resident Evil 4 was terrifying. I remember actually turning it off without saving the first few times because I couldn't bear to think what was maybe in those shacks on the forest trail, I have since gotten used to horror games. Dr. Salvador, I knew of him before I actually played RE4 (it took a long time to track down a copy) and when I heard that chainsaw motor I lost my shit, he's a damn bullet sponge! Scariest encounter is probably fighting him and then running out of ammo...

Last but definitely not least...

Big Daddies from BioShock/2: These aren't the usual picks for horrifying monstrosity, we know they're human and they're like a father hence the name. But the first few times you encounter a Big Daddy (and almost every time after you get used to them) you know that you are going to have to either come up with a brilliant game plan or think extremely quickly, they may look slow but once you attack them, shit will go down...the scariest encounter is every single time because it's such a carnage-filled brawl, but I'd have to say being low on ammo then encountering one sucks.

Pyramid Head

Silent Hill 2 still scares the fuck out of me many years on. I can't bring myelf to play it on my own, or at night.

I remember when I jumped into a sewer and the bastard was there, I needed to change my pants :'(

That is the most horrifying thing ever.

Honorable mention goes to the ReDead from Wind Waker. The ones from Ocarina of Time lost a bit of their creepyness thanks to Majora's Mask, where you can get them to burst into dance by wearing the right mask. And there's just something about the exaggerated, cartoony look of the Wind Waker ones that make them so much more horrifying.

Mine is from Resi 4, the simple cannon fodder zombies that, after being decapitated, GROW ANOTHER, BIGGER, FREAKIER HEAD. like a centipede with blades on it.
You tell me, when you've already blown a zombie's head off what do you aim for? all your zombie-killing strategies go out of the window and you just have to unload every bullet you have until it's dead. at least there's a reasonably simple way to kill regenerators, they're basic enemies that can take you completely off-guard

Honerable mention to a game called Dark Sector, your aiming is clunky and the camera isnt your friend, then the game drops you into a black sewer and you fight wave after wave of mutant redneck style things that can over whelm you easily. I think it helped that I was about as crap with the gun as i would be in real life...

Ghosts from Silent Hill 4. They like to follow you, and you can only pin some of them down sometimes, and even then they make horrible noises. Possibly the worst part, though, is the fact that they each have an identity. They have faces, clothes. They were individuals with lives, and now they mindlessly prey on people lost in living nightmares, possibly forever.

Honorable mention: Dead Hand from Ocarina of Time. Limbs sprawling all over its lair to grab you, and that horrible face...

These Las Plagas wolves are the bane of every moment I'm in that maze when I play. Regenerators also scare me because of that creepy breathing but they're easy to defeat once you get the infra red scope or one of the more powerful guns. For the Colmillo every single time I encounter them I get that shock of fear sensation when they appear and they are the ones that do the most damage to me. If the entire game consisted of mostly Colmillo instead of the other Ganado types we'd have more deaths in the game than you would missing a QTE button command.

Half live but not the poison head crabs the fast ones, don't know what they are called, The never really did anything other than die, but there roars and look always made me but crap my self.

The scariest I can remember was the Flying Polyp from COC: Dark Corners of the Earth. Not only were there two of the ugly, huge SoB's, I also had to fight them over a seemingly bottomless pit with the wind constantly threatening to push me off the narrow platform I was standing on.

Father Dagon was also a pretty impressive sight the first time I came across him earlier in that same game.

Tanks in Left 4 Dead, the fucking striders from Borderlands.

Also, the Helghast from Killzone 2, because there are hundreds of them, and they can all take a clip of bullets each before going down.
