ok this one is simple, what's the most unfortunate way you or an npc died in a game?

for me it was in cod4 where my friend tried to cover behind a brick wall whilst there was an enemy on the other side. the enemy then used his melee to send a brick flying into the camera and he died. although it was the knife that killed him it looked like it was the brick.

Of an npc this would be carmine in gears 2 and 1, mostly because of his stupidity

(yes i used the search so sorry if this is all done before)

When I beat an NPC to death on San Andreas with a dildo. That would be an unfortunate way to die.

Similarly to what JemJar said, there's a cutscene in Resident Evil 4 that pans over an area, showing you the layout and where enemies are etc. If you choose not to skip that cutscene, one of the enemies can get right up in your face. Happened me once, I'd been thinking "well, I'll heal myself once I get into this next area" <knife to face, death>

Zoey getting pulled off of a roof to fall to her death by a smoker. This was as disappointing as it was fucking awesome!

npc - Mickey in Fallout 3 when you're being evil ]:)

me. Playing duke nukem 3d multiplayer I get blown into the air by a pipe bomb then blown up in mid air by a lucky RPG shot. Awesomely fail way to die. (So awesome I can remember the scene from nearly 10 years ago)

I was not particularly disturbed by the level of gore in Resident Evil 4, but once I got Leon killed by one of those insects...

Ew, imagine being killed by acid-spewing giant bug ripping your face away...
I would not like to die like that.

On the Halo 3 Legendary campaign, I was doing the Cortana level (the last one needed to get the Legendary badge), I had crossed the last checkpoint and was walking on the elevated "bridge" to access the Pelican...and that is when the Arbiter (CPU controlled) killed me with the flamethrower
I was like o_0
Also in TLOZ:MM in the stone tower temple standing above the block to shoot the red jewel

Getting owned by a Tank on the rooftop finale on No Mercy.
Agreed.

Getting tank-punted off the hospital roof as my group made the final push to the helicopter was the single most frustrating gaming experience I can remember.

Ha. Fail

Other than that the most unfortunate death I have seen has to be Andrew Ryan. Damn those kinda twists gotta suck.
