It's been a while since I saw it, but if I remember correctly the main character was a resident of the institution. And he was in denial about killing his wife so the doctors let him play detective so that he could discover that he was the one that killed his wife. It's been a couple of years since I watched it so it could be a little fuzzy.

I can tell you. I remember it.

So, the Detective finally realizes that he's just an insane asylum patient who murdered his wife after she murdered his kids. The Doctor tells him they've made this breakthrough before, but he constantly reverted to his mental impaired state. The Doctor also tells him that if his experiment (The role play) actually works, then future Psychology could use that instead of the tried-and-true-but-painful Brain Operation. At the end, it looks like he reverted to his old state and they're gonna have to operate his brain. But then, the Detective asks his buddy (Who is a person working at the hospital) the question "Is it better to live like a Monster, or die like a good man?" as he's being taken away. This basically means that he didn't revert to his old state; the Role Play worked completely, but the Detective can't live with himself. He'd rather be a bumbling fool then a man that knows he killed his insane wife... However, that kind of makes me mad because the Role Play Worked, and instead of staying sane and letting others get help through this method, he acted insane and ruined any chance for the Psychological test to be taken ever again. Although, in the original Novel, he actually did revert back into insanity, so, there is that.
