Need help upgrading my PC

Hey, guys I've bought my PC (custom build) about 2 years ago and playing some of the newer games coming out this season I am gonna need a upgrade. I'm sure you guys can help me. Here are the specs

P6T SE LGA1366 Intel X58/ICH10R
Intel Quad-Core i7 920
DDR3 6GB 1600 Kit3 Gold 1.65v
SPINPOINT F3 1TB 32MB
Radeon HD 4890 1GB GDDR5
PSU Real Power 620W
Samsung F1 750GB HDD 7200rpm Sata 32MB

So in order to enjoy my graphics in the new gaming season how do I proceed?

I was thinking about replacing my Hard Drives (one fast, one large and slow) and a new graphic card, but you guys are obviously better informed then me at this stuff.

Probably just an updated GPU (Graphics), def' upgrade your Power supply to 750W+ and if you're looking for speed and stability, a 160GB SSD couldn't hurt (with SATA3 Cable).

Other then that, seems perfectly fine.

Thanks, any advice on a GPU for that matter? Or several for making the choice a little easier.
And what would I do with the SSD install windows/games on it? or?

@JochemDude, I honestly can't tell you Graphics cards as their all to each his/her own preference. I PERSONALLY would recommend a nVidia GTX 570 as their in the top 5 tier of current GPU's and are roughly $350-400. (ALL EVGA come with 3YRS Warranty)

SSD (Solid State Drive) is basically a brick design of a Hard Drive, designed to run things MUCH faster, and better lasting life. However they cost more then the usual HDD as it's designed for performance then storage space. It's not really needed but for better loading of programs, games etc it does help.
