Who can recommend a good RPG that does not take place in a medieval fantasy world, aside from the fallout games. I already own all of those.

Final Fantasy VII I consider the best non-medieval (though the Fallout series, BioWare's offerings and Deux Ex are all great), though it certainly has a heaping of fantasy elements.

Mass Effect is one of the only that comes to mind, although I will say that some of the Final Fantasies take place in a more futuristic setting (7 & 8). It really depends on what you're using as your definition of an RPG.

Hey stop eating apples. Oranges taste better and they help prevent scurvy.

On-topic, I'd recommend the Mother series (well, the latter two at least. Mother 1 is notable only for its music, really) as well as the Final Fantasy Tactics games (depends on how much you like TRPG and how you define "medieval".). On that note, all the FF games could fit your criteria. But seriously, Mother 3 and EarthBound, I cannot recommend these two enough.

System Shock 2 (and I suppose System Shock, though it's just too dated for me to play) is always a good bet, and tactical RPGs have never gotten better than the original X-COM (aka UFO: Enemy Unknown).

Mass Effect has kind of an old-school sci-fi setting. It plays a bit like a shooter.

Knights of the Old Republic 1 and 2 are Star Wars. Actually, they're really, really Star Wars -- many people said that KOTOR felt more like what they think of as Star Wars than the prequels did.

Jade Empire draws on Asian (mainly Chinese but not entirely) themes for its setting and inspiration. It's like kung-fu fantasy.

Arcanum: Of Steamworks and Magick Obscura is all, like, magic-steampunky. So elves and magic but also guns and mechanical stuff. I've never played this one myself.

Vampire the Masquerade: Bloodlines is set in modern times. It's kind of a "dark fantasy" thing, i.e. lots of horror elements but the story structure itself isn't horror.

Planescape: Torment has one of the most interesting fantasy settings ever, and, in my opinion, the best RPG storyline hands-down. It's rather old now, but if you can tolerate Fallout 2's graphics and interface you'll have no problem with Torment. Go play Torment now!

The Deus Ex games and BioShock are mainly shooters but have RPG-ish elements in them.

Eh, it depends a lot on weather you can get friends to sit around a table with you or not. If you can get friends, then D20 modern, the Star Wars RPG, GURPS future, most of the stuff put out by White Wolf Studios, Rifts and a bunch of the Munchkins sets from SJG. If not, then the other games advised above will suit you very well, maybe Deadspace, as well. Depends on how loosely you define "role-playing game."

Recommending Rifts to other people... have you no soul?

If you like JRPGs, then I'd suggest Persona 3 FES and Persona 4, both of which take place in present day.

I've played Arcanum, it's pretty good. I haven't played Fallout but I've been told the gameplay is extremely similar, if not identical. I got bored with Arcanum fairly quickly, but that's more because I kept restarting to make a "better" character and eventually bored out. I do that a lot.

Mass Effect was a decent 25 hours in my opinion. If you want my thoughts on it in-depth, I wrote a review [http://www.escapistmagazine.com/forums/read/326.73854] a while back.
