Struggling to be open and vocal around people doesn't necessarily mean something's wrong with you. Being shy and uncomfortable in social situations, or around people you're unfamiliar with doesn't automatically mean you should worry, many people struggle with this. This is something you can work on improving, you just need to decide that you're going to do that. As for the shapes, although you may see them, it's possible that at this point you're focusing on it which is only making you notice these sorts of things more. The more you tell yourself you're seeing these things, the more likely it is you'll see 'em. Lastly, these sorts of things could be a result of an active imagination, and/or the amount of sleep you get/how much you eat daily, both of which can be corrected with a little determination.

These results don't mean anything, it's an incredibly simplified test. The more you focus on it the more you may convince yourself that you've actually got something to worry about, when in fact it's not likely, at all. If what you listed above worries you, that alone should be reason enough to talk to someone. As already mentioned, for peace of mind more than anything. Plus, it can't do any harm, there are no negatives, so why not?

I would suggest getting into the habit of eating a minimum of three meals a day, but preferably four or five smaller portioned meals. Breakfast is as important as everyone claims it to be. The sooner you eat after waking up the sooner your metabolism will get going again (it slows down while you're sleeping). Also, assuming your morning is between 7am and 9am, I'd suggest snacking between your breakfast and 2pm meal. Lastly, the fact that you're not eating much from dinner to the time you go to bed means you're that much more hungry when you wake up. What worsens that is the fact that you only drink coffee in the morning. All of this isn't good for anyone.
You're concerned about the things you've listed in the OP, so why not talk to a doctor? Either you'll be reassured there's nothing to worry about (while getting some good day-to-day advice) or certain things will be pointed out to you, and you'll be able to work towards correcting whatever comes up.
