But personally, I prefer the Creative Assembly making historically accurate games, while modders should make these kind of spin-offs.
TW... 'historically accurate'?! Pfff...
OK, to be fair they're... mostly accurate, though often dripping with anachronisms. I play mods to increase the historical accuracy (though right now I'm mainly gunning for R:TW).

I'd much rather see a Total War game based on A Song of Ice and Fire.
I like Elder Scrolls but it just wouldn't work in this kind of game.
Likewise...

What makes TW great and what makes ES great are two tremendously different aspects of gaming that (thus far) are virtually impossible to combine into a single gameplay system. You lose all grandeur of scale (specifically of settlement expansion and combat) in the 3P mode and all control and empathy with the character in the strategic mode. ES has always been about one man/woman/thing changing the world by his/her/its own efforts through extensive interaction with the contents of said world, while TW is detached from all that guff, it's about raising massive armies with ***** units lead by hard-asses and taking over the world for the glory and teh lulz. There is no overt good/evil in it other than pure ambition in TW, while ES is about fate.
If you're going to have a specific TW game of a fantasy universe, CA will look to fellow Brits (solidarity and all that, what?!) at GW with WHF. Granted a mod's already about for it, but given the nature of the source material, game and scale, it's so much more suited to the TW type of game.
Based on the game lore, the way that ES is run, I think it'd be better if such a game was a pure RTS but in the mold of say...
Supreme Commander: character spawns somewhere; starts hacking at enemies; gets allies; commissions buildings to be built (i.e. mercenary outposts, magic schools, armourers, guilds for certain units/characters etc.); objectives get further away and more difficult thus ramping up the challenge and the necessary number of combat effectives. Or something like that...

Option 1: Yes, I would diffidently buy that game.
How would we go about that?! Do like Fluttershy while we're in the shop?! =P

So, like Star Wars: Empire at War. 4X with real-time combat.
Sort of... more like the Galactic Conquest mode as far as strategic/tactical segregation is concerned.
