Batman hallucinates that he is back at the beginning of the game, but he is being wheeled into the Asylum and you temporarily take control of the Joker. After a while, you switch back to controlling Bruce, but from the first-person perspective of him strapped to an upright gurney. Joker puts a gun to the bat's head and pulls the trigger. It kills him instantly and sends you to the screen that asks you if you want to quit or retry, saying that you needed to nudge the stick to dodge the bullet. No symbol or anything. Upon hitting retry, you are sent to a totally different part of the game where Batman continues his hallucination. I thought that was a very good mindfuck myself.
