In the case of these people you speak of, the problem isn't primarily that they're smoking - it's that smoking is their only way to relieve stress. Being in such a situation, not to mention that I'm sure plenty of people around the globe are in such a situation, is an arguably worse problem, and it's closely related to the reasons behind the decision to continue smoking. If society collectively realised that we'd all be better off if no one smoked, we'd get one step closer to bringing out some measure of caring for one's health from many individuals.
The same logic can be used to motivate taxing 'restaurants' that serve particularly unhealthy food.
