In your opinion, has there ever been a 'perfect game', with no design flaws?

No.
There has, in my opinion, never been a game that has perfected what it has set out to do.

Some have come close, but I doubt there ever will be one to truly master it.

Impossible. I think it'll be a sad day if there ever is a "perfect" game. Also how would you define "perfect"? Because there are so many genres and different rules for each genre.

Yes there was one flawless game; The Legend of Zelda: A Link to the Past.

I would have said Ocarina of Time, but that had Navi...

My perfect game would likely be boring to the average player. Sadly my perfect game requires online multiplayer. So essentially no.

I have never found a game that was exactly what i wanted, and if i ever managed to make my perfect game it would probably fail because the average person probably wouldn't like it.

A flawless game (IMHO) would literally have to provide potentially endless entertainment to everyone who ever plays it.

People being, well, people this could never happen. Someone will always find something they don't like, could improve, could change about any game. Some people just don't like certain genres or styles of game.

With so many differing opinions on just what constitutes a flaw there couldn't have been a perfect game that could be considered flawless by everyone who plays it - nor should there ever be. We must consistently identify and look for flaws in games so that the next generation of game can be improved and so on, and so forth for ever. A sad day indeed it will be when someone does invent the perfect game - because then there truly will be nothing new to discover and the entire industry would collapse. Why ever buy and play another game then you've experienced the 'perfect' one?

ANYWAY - long, badly written rant aside I don't think there has (or will ever be) a 'perfect' game, but for me - the perfect game would possibly be an RPG that would be able to balance itself based on my play style and react to my decisions as a level-headed human GM would.

The one reason? People, no game could please everyone.

There are a few games that I believe to have been flawless; as in, if there are design flaws, I don't know about them.

1. Super Mario RPG
2. Super Mario Bros. 3
3. Final Fantasy Tactics (translation errors do not count as design flaws)
4. Tetris

I'm sure there are more, but I will limit the list to these titles for now.

No.
However the only two to be close to perfect (for me, at least) are Starcraft and KotOR.

The only perfect game has no release date. If they have to stop making it so they can actually sell it, it will have compromises.

The first problem with all games is that they contain bugs and glitches which could give an unfair advantage either for the player or the NPCs, since my time playing computer games for over a decade I have at least experienced one problem of this type in every game I have played

then the second problem is targeting to an audience that everyone would enjoy, not everyone has or will enjoy the same game until every human has been assimilated by the borg
