Well, you have to keep in mind that the other is the one dearest to you, not just some girl.

Could be your girlfriend, Mother, etc.

Depends how many innocent people there are, how old they are, and whether or not I like their choice in clothing. If I wasn't bound by your arbitrary constraint, however, I would snap the neck of the villain, throw his body underneath the falling girlfriend, and web the cable car in place. Then, assuming my lady love hasn't already landed in the villainous corpse's tender embrace, I would leap after her, snatch her out of the air, and curve myself so that she lands on top of me. I mean, Bendis' Spider-man could probably pull that off.
