I put it over, and if it isn't over, I end up fixing it. Luckily, the only person to do that is my sister, and she's usually to lazy to replace the toilet paper, so this situation rarely occurs.

And if i did have one, as long as its close by i couldn't care less about how it was folded.

Well, my holder's broke so we just sort of keep the TP piled high near the toilet. That seems to work fine enough.
