Middle school I gues, Twas during my massive online gaming days that I learned how to focus on the screen and not on my hands.

heh, heh, funny story. at the time my class was learning to type (which was elementary) I injured my right arm badly. had to learn on my left hand.

now, I can ONLY type on my left hand. it's mentally challenging typing with both hands. funny, eh?

Learned in 5th and 6th grade. I still have a fairly good speed, but nothing compared to one of my friends, who had 120-ish wpm. It was ridiculous watching him type.

I can type fairly well with my periphery vision, so I'm always watching the screen at any rate. I used one of those typing programs when I was about 14 or 15 I think. Didn't get much help from a teacher or anything, but I'm decent enough. About 40 wpm if I'm pushing myself, though I make an insane amount of typos, all the time.

I've typed since I was probably 6 or 7, so I've become quite quick. However, I did not learn how to type without looking until I was 14 or 15 and then "proper" typing (forget what it's called) when I was taught in high school.
