In Oblivion, I used a spell that paralyzed me for a minute... and I melted. I actually became a pool of player character flowing down the side of the mountain I was on. It was the most spectacular alchemical triumph I've ever accomplished.
I was playing Fallout 3, and I had brutally murdered the doctor in Paradise falls and was ready to begin looting. My first move was to shoot her in the head, you know, kind of like teabagging, just making the head gib. Then I turned back and grabbed a stimpack off the shelf. And I hear, "Hey! That isn't yours!"
I looked around, No one. Shrugged it off and opened the doctors first aid box.
"You theif! But that back!"
Agian, searched the building. No one. (It was only two rooms big)
Looted a bunch of stuff of the doc's shelves
Agian, rambling of "You theif!" etc...etc....
My conclusion was that a dead body was talking to me. Fun glitch, kinda creepy.

I remember playing Rainbow Six Rogue Spear and we spawned into the Opera level, we sat and typed for like 20 secs outside the building, telling everyone where to go (it was a coop tango hunt.) And then out of nowhere, the building freaking starts emptying all the tangos out into the street with us. Horrifying.
Another would be when we were modding some Bf2 stuff for our mod (Department Store Battles) and I somehow ended up falling through the ground, and just kept going and going and going and going. I fell for like 2 minutes before I suicided.

My friend was on the first island and I was messing around, and then while running I ran through a wall and into a tunnel which lead directly to the next island, which was currently blocked off. It was like discovering El Dorado. But then, in my excitement, I drove out of the tunnel and into the sea. This being GTA before people could swim, I died and respawned back on the first island.
