I once played some horrible PS2 port of Resident Evil 4 on my PC.
Couldn't get past the first QTE because I couldn't remember which keys corresponded to the buttons of a PS2 controller.
Gave up after 2 minutes. xD
If it makes you feel any better, I played that on the Gamecube when it came out and they thought that after someone failed a QTE it would be a good idea to switch the button you needed to press to something else for no reason at all. Pretty much guaranteeing that if you failed a QTE once, you failed it twice.

Fuck that game.

I think the absolute worst QTE I have encountered was the torture sequence in Metal Gear: Peace Walker. None of the other Metal Gears had a torture sequence this difficult. You have to mash the A button so damn hard and for so damn long that its ridiculous. Well maybe in Metal Gear 1 but you had the option to just give in and get the bad ending. Peace Walker doesn't give you that option even, it just kills you and sends you back to the beginning of the cutscene. For a stealth game to stonewall all your progress until you become a master button masher is stupid.
I ran into this problem when I recently bought MGS1, I had to use a mystic spoon technique to get through the torture bit. I agree that it doesn't really sit well in a stealth game like MGS, the same could be said for some of the boss battles in that game. I would have loved to just skip some of them and get back to that stealth goodness, especially that fucking awful punch up with Liquid.
