I don't think there is much risk of this. The interior of the towns and hence the population of the towns don't even load up unless you click on the entry doors. Named-NPCs dying and unique encounters going unnoticed as you fly speedily overhead. It really is counter-intuitive to the nature of the AI encounters in the game, but I would still love to do it... sadly, after milking the entire game dry of content so that I don't accidentally spring something and end up missing it, or flat-out ruining my chance at it later.
Not everything happens within town walls, plenty of named NPCs hanging out in front of caves or forts, just waiting to be swarmed by the army of Forsworn you've been trailing behind you since you flew over Markarth.
