The ending of Metal Gear Solid 3 is one of the saddest things I've ever seen in any form of storytelling medium. The revelation of all the information about The Boss and the machinations behind the story is just heartbraking, but as soon as it got to the "She was a true patriot..." line I just broke down and cried.

The ending of The Crow also tends to make me pretty sad, largely because of Brendan Lee actually dying while making the film.

Apart from these, the ending to Evangelion (despite being all kinds of "off-my-meds" crazy) was so sad I'm surprised it hasn't come up yet :/

You mean the show ending or the movie ending?

Eventhough I knew Grave of The Fireflies was going to end badly the moment it began, the ending still got me.
