Didn't they use an actual cow fetus, like a mold of it or something, to give it its realistic look? I think I heard something like that.
They always kept it secret how they made that puppet. But it seems likely that it did involve a dead animal at some point.

So you're a George "Man is the real monster" Romero kind of guy? :D
Kind of true though. It's hard to beat just how repellent a character like Captain Vidal in Pan's Labyrinth is, when they don't even have cartoon villainy to stave off the discomfort. For much the same reason, psychological horrors like Dead Ringers or Saint Maud are in many ways more disturbing than supernatural horrors.
