Basically it has these little parasites inside of it (like 4-5) that you can see only with a thermal scope on your Sniper Rifle. Which means if your out of rifle ammo, you just sorta spray and pray and hope you hit the parasites.

It was really scary because you tried to stay calm and aim straight, but they just slowly walk towards you and they rush me and make me miss lol

Either the zombies from Vampires the Masquerade Bloodlines, the Imps from doom 3 (you are walking along and out of nowhere one leaps from behind a door, in the ceiling, or out of the freaking stairs), or Facehuggers from Aliens Vs Predator (1999)....they were so small and I could never see 'em, and I would always jump out of my seat when one latched onto my face

Shades from Fear Extraction point. When i first played that game i played it on a crappy PC with all the graphics on Low, which made them pure black instead of transparent. I think it made them more terrifying, as you walk into the room and the lights go off with your torch the only illumination as you began to see shadows move in the corners but disappear when you look. Then your torch splutters and dies...SHIIITT

I know what you mean, it scared the crap out of me when I was smaller. I'm still scarred to this day.

The woman from the blinding ritual in Project Zero (Eu) / Fatal Frame (US)

Prepair to say "OUCH!"

Highly recommend this game though for someone looking for something diffrent other than Resident Evil or Silent Hill.

Just to note the woman that has this done to her is an enemy and boss that pops up at the worst times to make you jump

Yeah, boy! That fucker freaked me out when I first saw in in #2. I didn't know what I was seeing, but I knew it was phucked up.

Really? yeah they are annoying and a pain when you get 50 running at you, but they ain't scary, they are more funny if anything, they run at you SCREAMING without heads and bombs in their hands

Took a full Broken Butterfly to the head to take him down, and that was on normal. I still need to finish that game.

I was playing Ocarina of Time, and was in the water temple. I was borrowing a friend's guidebook (because it was the fucking water temple), and the jerk thought it would be funny to mess with me, so he scanned a page and replaced the text and pictures (believably for the time, fifth grade I do believe). It said that the dragon pipe that holds a switch would come alive after I used the iron boots to sink to the bottom. I hated battle under water, so naturally, I did not want to do this fight. And it freaked me out, to the point that I stopped playing for a week. When I finally built up the confidence, nothing happened, and I bitched out my friend the next day, who just couldn't stop laughing.

Another one would be Nemesis, though that was mostly due to how intimidating it was, not how scary it was. And the mannequins that follow you in the original Condemned would have gotten me had I not been expecting it. But since we're on Condemned...

There's a few honorable mentions.
I agree with the bear. Terrible.

I'll have to say that the scariest monster was a mini-boss from Silent Hill 5. You're stuck in a small room, and he's got wolverine hands. Your advantage? His eyes are sewn shut.
