They're incredibly strong, shuffle towards you slowly, regenerate limbs (eluding to the name, no doubt) and are only killable if you know where to shoot.
Damn that thing's ugly. Where do you shoot to kill it?
Basically it has these little parasites inside of it (like 4-5) that you can see only with a thermal scope on your Sniper Rifle. Which means if your out of rifle ammo, you just sorta spray and pray and hope you hit the parasites.

It was really scary because you tried to stay calm and aim straight, but they just slowly walk towards you and they rush me and make me miss lol
I forgot to pick up the Thermal Scope on my first playthrough, it made RE4 much more scary.

For me it would definitely have to be regenerators from resident evil 4 both the regular and Iorn Maiden versions. Two of the scariest moments Ive had with them is the first time I faced one when you hear the loud bang from the other side of the lab door but the second time is much worse becouse I was in a narow hall way and you cant see it but you can hear it then as I start walking I turn the corner and its right there less than 2 feet infront of me I nearly craped myself.

Invincible Necromorph from Dead Space... Anytime it showed up, I turned off the game and walked away. I hated that thing with a passion and every freaking zombie I saw made me wonder when he was just going to pop out of some air vent and kill me.

The Anglers in Aquaria.

I hated leaving the safety of goddamn wall* anyway, but leaving the wall, in the darkest depths of the fucking ocean, only to smash headlong into one of THOSE horrifying things? I'd shit my fucking pants.

*ANYbody who has played Aquaria will know what leaving the safety of the wall means.

The Nercomorphs probably, I didnt exactly find them that scary, but when one is dragging towards you desperately trying to get up to you and rip you apart, its just kinda horrible seeing what they are and what theyve become.

From an actual combat perspective id say the STALKER bloodsucker, like Zoidberg but not so cute, and damn creepy.
Oh yeah and it runs around really fast in the dark, then TURNS INVISBLE and eats your face.

Ohhh the headcrabs from HL2. The first time you smash open a crate in that small little tunnel and it comes crawling and screaming at you...I think I actually yelled.

Oh and the whole city of Ravenholm. Those little buggers made the game very hard to play late at night. However, they do make a very fashionable hats.

Speaking of late at night...is this really the best thread to be reading at two in the morning?

EDIT: Oh and all of the enemies in that old Arcade Game "CarnEvil"

For those who can't see, that is a giant infant you just blew up.

With lightning.
My arcade has that game...i've still NEVER beaten it, i've gotten all the way to the RingLeader at the very end, but never ever beaten him.

That game has earned so many of my quarters.

Tonberries. No, really. I tense up every time I face off against a tonberry in a Final Fantasy game. Cactuars and any enemy that has an attack that brings you down to less than 10 hp to a much lesser extent.

U-3 from RE4. I think I quite literally pissed myself when he appeared. I spent most of the fight shrieking and yelling "NO NO NO PLEASE NO!!!"

They....hump you to death...far more terrifying than what most zombie-like things do to players. X_X

Hm from a half-life 1 rpg mod, Master-Sword, there were some certain dungeons and caverns with a particularly fightening monster, i think it was some sort of demon but it was tall and red and moved like the wind, the caverns were dark and winding and they'd literally come out of the walls and hit you, and then run away and be gone in an instant, but you can always hear them ><

The incredibly fast invisible ninja enemies from the original F.E.A.R

Crawling around the walls and roof, fading in and out of visibility and being FUCKING FAST!!! The first time I saw these things I went paranoid and became incredibly trigger happy :p
That's the ONLY part of the game that I was actually freaked out over. Then I was disappointed to find that they only appear in that office and then again near the very end. That still didn't stop me from being paranoid the rest of the game.

Mine is the Hall Worms from Deadspace. So far it's the only in-game event to actually cause me to simultaneously scream and fall out of my chair. It's just so goddamn unexpected every single time. Though I'm playing through Penumbra and there have been some close calls. More whimpering and hiding in a corner with that game though.

I think I was finally able to beat all three (or whatever) campaigns but every time I played I must have spent at LEAST $5. That old arcade must have $30 in quarters just from me and my friends.

Yeah it's frustrating! The whole thing! I'm there with you! They should either remake it or release it on another system. Then again, it may not be as good.

Don't know if anyone here has played the game Vampire The Masquerade: Bloodlines but the Tzimisce creations you encounter in the sewers scared me the first time through the game; just because they lunge at you and they are small and hard to hit.

Any of the Giant Spiders from the Resident Evil games.

I don't like spiders anyway but the idea of them being big enough to tear off my head with their mandibles of terror just gives me the heebie jeebies.

The fast headcrab zombies from hl2:

It was late at night and I was completely alone in a very large (and 100+ year old [read: creaky]) house, had just sawed my way through dozens of regular zombies with nothing but the grav gun and crowbar, when I hear a godawful screech, and see this beast lurching horribly towards me at inhuman speeds.

It's even creepier when you look at it up close, and you see the muscles and bones, where the flesh was literally torn from the still-living body, likely by the person's own hands, all while they're completely unable to control themselves; only play a horrid part in their own torture, like a puppet who's master wants only to see them suffer.

Also the screams that the normal zombies make when they're on fire are pretty gruesome as well.

P.S. To top it off, the next night I was meeting my girlfriend after work, and on the way, under a burned out lamp post (almost pitch black), I heard the exact screech of a fast zombie. Suffice to say the walk home was more expedient.

To be really honest, any enemies in the Fallout 3 of TES:Oblivion games.
When you're simply strudding around and suddenly hear something, turn around and freak the fuck out because there's an enemy in your face. The sudden shock just always gets me.

Alma, but that's already been said...
And lastly the spiders in Dark Messiah: Might and Magic

You mean those creepy little bastards with only two legs that run incredibly fast and make that horrible dry hump noise when they attack? Bloody hated those things.

Surprise encounters with Feral Ghoul Reavers in Fallout 3 are almost guaranteed to scare the crap out of me. :(
