No one's gonna agree on first impression, but...

King's Quest 4.

It's not an actual horror-FOCUSED game, but damn is it scary. There are literally about 5 dozen ways for you to die, and this is in an ADVENTURE game with sluggish movement controls. You walk into an area and there's an OGRE THERE IT'S COMING AFTER YOU RUN. You pretty much get no weapons through the whole game and just have to hope you know how to deal with a monster in the few seconds it gives you before death. And then there's spiral stairways you have to navigate with said sluggish controls. One pixel of you stops touching the stairs and you fall all the way down to your death.

Quicksave-crawling is the only way to beat that game. (plus a GameFAQ's guide)

I'd recommend Call of Cthulhu: Dark Corners of the Earth by Bethesda. The downside is that it's filled with useless shenanigans and a wonderfully lame transition from detective-type of gameplay to poorly executed FPS. Other than that, the game is tense all the time and builds up greatly. I played it with god-mode turned on most of the time but it was still quite scary; not in the usual survival horror sense but because it has such an oppressive atmosphere.

Picked up a quote from another thread:

Call of Cthulu.

If you ever want to feel powerless in a city that hates you, this is the one.

"Vampire the Masquerade: Bloodlines" indeed, there's some suspense/horror in "Call of Cthulhu: Dark Corners of the Earth"

I don't know many really good horror games that actually deliver.

But this cute little flash game is pretty scary when played in dark silence.

I wouldn't really call Bioshock a horror game, but whatever.

If you like scary games, then you NEED to play Eternal Darkness: Sanity's Requiem.
Bloody brilliant game. If you still have a Gamecube or own a Wii... you must play it.

The Path [http://www.escapistmagazine.com/articles/view/editorials/reviews/5928-Review-The-Path] is pretty scary, although not immediately and incredibly subtly. It's not for everyone, but it's very cheap, so you should at least give it a try.

The first two Penumbra games are pretty damn creepy. Psychologically sometimes.
