Should it?
The four options are:
None- No Gore.
Same- Fallout 3 Cheesy gore.
Improved- Cleanermore realistic gore.
EDIT: Shit, son. Sorry. I was going to add more, but the creature that is The Escapist fucked it all up. Also the poll is missing.
Fourth Option- MOTHAH FUCKING OTHER!(possibly a mid-ground, or on/off button).
WELL. I personally like the cheesy gore, because it makes me feel like I'm in a cheap 60's-80's B-Movie

You said "four options" and only listed three...

That being said, I didn't mind the gore itself in the game, but rather how I was forced to watch it over and over in slow motion. The first time someone's head is blown to bits by a VATS crit is cool, but when it happens EVERY DAMN TIME you shoot someone in VATS it gets kinda dull. It stops being "OH SHIT, I just blew that guy's head off!!!" and becomes more "Meh, whatever". I actually downloaded a mod that dropped the occurence of gory kills down to 20ish% (or some such number), not because I couldn't handle the gore, but because it got boring.

Skyrim has a slightly better approach here - decapitations are in the game, but they aren't very frequent and therefore stay cool. You don't decapitate every dude you meet, but when you do, it's pretty awesome.

I don't really care if they make the gore more realistic (personally, I think it's wasted effort), but I do hope they don't make it boring and ordinary...

I don't care, just as long as the death animations are as hilarious as they were in Fallout 1 and 2.

No more decapitations by pistol bullet. Guns do not work that way.

I can't decide whether or not it's because their crappy engine can't differentiate between weapon types, or whether they were so taken with the near pornographic gore shots they decided to just apply it to everything.

The reason the Fallout 3 and NV-gore was so ridiculous was because it's an honoured Fallout tradition. Yes, it did get rather ad nauseum at times, but that's what traditions do.

I think they should keep it. Fallout have never done much of an effort towards being realistic, after all, and the inherent cheesyness of the whole thing is the main spark in its peculiar charm.

I'd prefer if it stayed. Or even have it on a scale in the settings. If and when it is released I want to be able to horribly mutilate the corpse of the game's equivalent of Roy Phillips.

Depends... how many people here took the 'bloody mess' trait for the extra 5% (or whatever it is damage?) that might make it appear more frequently than you realise, As for the gore porn... meh... i just took it as silly to be honest.
