I've promised science that it can have the parts of my body they are interested in. What few parts that are left outside the lab will be cremated and buried to the following tunes.
My headstone/mausoleum (that depends on how rich I am when I go.) will say something along the lines of: "I intended to live forever or die trying..."

I want my body to be sent into space. If I can't manage it in life, my corpse will at least boldly go. Maybe into the sun. That would be cool.
The flawed part of this hypothetical situation is where it implies that I can die.
Still, I'd likely have myself resurrected as a more appropriate - if moderately more odorous - leader of my army.

I want a small granite gravestone engraved with an ouroboros on a cliff overlooking the sea, with my rapier. I want no memorial besides this.

Donate whatever organs possible, burn the rest, have family and friends organize some event for celebrating my life/mourning my death, have something physical built/placed to be remembered by (Not a statue, but not a traditional 'tombstone' either). Done.

If I've got to die, I want one of two ends of the spectrum. I either want to leave a legacy so glorious that it's entirely incorruptible...to be a modern Beowulf, in a sense.
The alternative is to just fade into anonymity willingly, by having no memorial and keeping the memories of me to just those who deserve them, IE the people I knew in life.

I would love it if everyone just gathered around my open coffin and instead of a eulogy I would really appreciate it if somebody simply said "Hey look! He's moving!"
If I going to be buried, I want my possesion (games, TV etc) to be buired with me. Take that love ones! You ain't getting nothing from my will (well except my three closest friends)! Also I want a fail safe tunnel in my coffin if I somehow come back alive inside my coffin for about a year. After that year pour cement on top of my coffin incase of a zombie apocalypse.
