Insult to injury is added if he has the Eyelander, causing him to gain a health and speed boost from your failure to kill him in time. It goes against the nature of melee weapons, as well as against the purpose of demomen, removing their trap-setting abilities.

Ah well, at least I've decided on becoming a career pyro though, so I can just compression blast demoknights away as I whittle away their health.

I think knife-instakills are realistic.
If you get your neck slit open, you will die instantly.
And I'm sure real soldiers are trained to pull out their knives quickly and put them away again without dropping their weapons.
Buy that token, you get shot in the chest and you are going down. Not dead (necessarily), but your ass is going to met ground. And no, they don't die "instantly" when you slit the throat, they generally clutch their throats and make a gasping weezing sound as they try to plug the hole that is now in their throat. It's defiantly not quick and clean like the movies make it out to be.
If you notice in MW2, for example, they DO die clutching their throats, so meh.
And not necessarily about 1 bullet in the chest. It's like people being shot in the stomach - they quite often are able to stumble around for a bit and do things.
slashing someones thought wont kill them unless you hit one of the arteries/veins in the neck (very likly) even then they will still take half a min to a min to die and if you miss any artieries you just effectivly gave them a trachiotimy which deffinatly wont kill them, a knife at the base of the skull at the back however is a instant kill

its easy to pull out a knife at speed with one hand.

Now putting it back in at speed and your more likely to stab yourself in the leg.
