Random dude in elevator action i think. I played games before that but i don't think there was any "killing" involved in Jumpman or Hotwheels, or E.T. And the pac man ghosts were already dead...as well as the ghosts in ghostbusters on com 64. SO yea...thats my best guess.

I'm not entirely sure, but it was either the first goomba on SMB1, an enemy MiG on Super Strike Eagle for SNES, or indeed, as this man said:

Splat goes the Goomba!

Not on the NES, mind you, I'm talking Super Mario Land on the first Gameboy.
