I initially thought it was because originally they were going to have the screamer but replaced it with the boomer later in development, so I thought that maybe the intro movie had been made before the change, when they were still debating it. But yeah, it does make sense the boomer bile being what Bill wiped on Francis at the beginning.

The opening cinematic is brilliant in my eyes. Mainly because it does what so few games actually do - encompass the ingame mechanics and sounds into a cinematic - take the way Bill was captured by a smoker and then freed, or the way the tank moved - those animation seemed as though they could have been in game...in fact they probably are.

What more of a backstory do you need? Zombies have attacked, these poor people are trying to escape.

There is/was a story but they cut it because it would kill replayability :)

What's interesting about L4D's story is that it really is "there" - it's all in the character's dialog which is usually random, but still triggers at certain points, or it's written on the walls or found throughout the environments. It's a "streamlined" story, and the worst part is, no one knows it's there because you're too busy shooting the fuck out of zombies to hear any of the dialog, or look for things other than a zombie or special infected.

Well, in the developers commentary they state that the campaigns were supposed to be a sequenced story, starting with No Mercy, but they ditched it after play testers found out that the entire run to Mercy Hospital and getting rescued by the helicopter felt... unrewarding, seeing how it would crash and you'd be all over it again, only somewhere else.
I mean, that sounds even less rewarding than the system they got now. : P

It shows vomit at the beginning that's enough. And the story says that 2 weeks ago there was an infection, now everyone's a zombie, so you must kill them. That's better than most zombie games / films go with.
