After reaching a high score on an old arcade machine and rushing the initials 'A.A.A.' into the machine, it came to my attention. I have never used a name besides that.
Anyone else got other names they regularly use in games?

It depends entirely on the game I'm playing. I always used P.J.J. for my high scores in Starfox 64, S.P.F. for fighting games, Z.L.G. for shooters, J.D.V. for skating games and K.A.Y. for racing games.

It's always been Stevato or Steve-o for me, a lack of imagination perhaps?

I always liked Sir Noobalot though, i saw that name used regulary on CS:S

Swears. Or compounds of the most disgusting bodily functions I can imagine.

You should have seen my Civilization Revolutions map. Do not, I repeat, DO NOT swim in those rivers!

The most common names that I see involve the words 1337, shadow, ninja, sniper, pwn, and usually contains multiple X es before in the middle and after the name

Well if I'm able to put a full name, I normally use my last name. Nowadays I use my username, mainly because it's unique. Other than that I use my initials.

Killer or Killer+[Insert word]
1337/1337+[word]
I don't know, for some reason people choose a name with numbers after it in CS even though you don't have to register a tag... I think people name themselves Kevin153 in other games because Xbox won't let them just be Kevin.

I go by SuX2bU and I've only ever seen one imitator and he was literally an imitator because he played at a server I was on all the time.

In MMO's is depends on the race/class combo.

Or various popculture references.

Take my pally Macgyveer in WoW as an example. He isn't the first toon I named with a variation of MacGyver, but he is the last.

Xx1337xSn1p3RxX or some variant.

Let us break down the name in parts, shall we?

Xx usually means the main name has been taken in some form. Already a bad sign for the name's originality.

1337. These people usually are not 1337.

x. NowXyouXneedXtoXgoXputtingXlettersXbetweenX,XwhichXisXjustXsad.

Sn1p3r. Not only was Sniper, Sn1per, and Snip3r taken in the exact same position for letters, but now you had to substitute numbers into your name twice. Again, a bad sign.

xX. Put at the end to make it symmetrical. Which makes it cooler?

EDIT: As for myself, prefer Geohound Ryudo or Onyx Oblivion of some sort. So, either the main character of Grandia 2, the most underrated RPG series ever, or Onyx (which I think sounds really cool) and my favorite game of all time, bar none (I sunk 2500+ hours of my life into that game).

Something related to halo, sniping, owning, noob, or L337 sp33k.
I tend to use Birds in names like Chaoshawk, Da NightRaven (The retarded batman) RoflHawk and NoobEagle

Pretty much sums up every name on XBL. I've seen a few making an allusion to intercourse with 'your mom', and several containing some drug reference ending with 420.

Only creative drug-related name I've ever seen is etherbunny. At least it's somewhat clever.

I use either Hexadecimal16 or Ford Prefect pretty much everywhere. Usually the former since Ford Prefect is often taken.

Are we talking NPC or usernames (i.e. Xbox 360 gamertags)...?

For NPCs, I wouldn't say there's a consistently used name, but anything beginning with a title such as Doctor ... or Mister ... seems common. There's like five doctors in the Half-Life series alone lol.

For gamertags/usernames, I see far too many people with Xx[insert name here]xX. I don't think 1337 is as popular. They're both bad, though. But congrats to Onyx Oblivion, Neurotoxin, and notoriouslynx (some other guys who commented here) for using an X in an acceptable manner.
