The two scary video game characters that I can think of right away are the Little Sisters from Bioshock and the poison zombies that throw poison headcrabs at you in Half-Life 2. First of all, the Little Sisters don't look that scary, but their voices are creepy (plus they drink juice they extract from dead bodies!) Also, in one of the Half-Life 2 episodes there's a part where I came across a locked door with a chair propped up against it. Through a small window, I could see a shape running back and forth across a room very quickly. I took out the chair and waited for a second and a poison zombie broke down the door and started throwing headcrabs at me. I couldn't play Half-Life 2 for a few hours after that.

Gage Creed, Church, Timmy Baterman, Spot, Zelda Goldman, Victor Pascow and the zombified Rachel Goldman in the movie 'Pet Sematary'. Nothing ever scared me more in my...whole...life.

Any character from Gregory Horror show, after you've stolen their soul/dreams - can't quite remember what it was you stole from them in the game.

The whole game is incredibly creepy and twisted in its visual style, it's a bit like a cartoon and the characters have big square heads with very disturbing drawn faces.

Once you've captured someone's soul/dream and they see you in the hall, they will chase you all over until you either make it to your room or you hide somewhere else. The hotel's really big, bloody confusing and everntually you can't turn a corner without seeing one of the many characters you stole their whatever from.

Silent Hill, most definitely.

Also, to all the people saying Alma; she's not, though. Little girls are innately scary. Put their hair over their eyes, make them walk slowly and bang - you've got a freak show. It's not actually scary, just a cheap trick. An overused trope.

Shodan from System Shock 1 and 2. She's pretty evil in the first one, but downright insidious in the second.

If you've played the second, you can relate when I say she destroyed my world once I got to deck 4. No game with ever throw me like that one moment did.

The Regenerators and Iron Maidens from RE4

I dropped my controller in shock from when I got killed by one of those things.

And Pyramid Head.
Do I really have to say anything about him? He rapes other monsters and can kill you in a second!

Honestly the one thing in games that creeped me out the most and still does to this day......... that god damn radio in Silent Hill, not the monsters or the town, the radio.

RAAM in Gears of War. He's probably so scary because he's huge, cloaked in black and never really speaks except to hiss "Diiiiieeee!" or "Serve the queen!".

The Regenerators from Resident Evil 4.

the scariest sons of b****es you will ever face, EVER!!!

and if you thought they were bad on normal difficulty, try doing it on professional with nothing but your motherf***ing pistol!!!

other than that, i'd have to say the bear on condemned 2.
