lets put it this way, soemtimes in Skyrim you get attacked by assasins, and they drop a note.
Mine reads
'As instructed, you are to eliminate Rick Astley by any means necessary. The Black Sacrament has been performed - somebody wants this poor fool dead.

We've already received payment for the contract. Failure is not an option.

- Astrid'
You know, they'll never give up. They'll never let their clients down. And I hear they'll never turn around and desert you, either.

Depends, if it's High Fantasy my usual "go-to" names are Azarias and Everius, with possible variations on them or a new one if the character in question is very different from something I've played before. Not-as-high-fantasy or sci-fi I usually go with names such as Richard, Rufus, (or in the case of female characters) Diana and Michelle. One of my favorite characters (and names) is Kalverian Kehieleth, but it's hard to make that fit to a character beyond the original :p

Probably my favorite name is Cadius, which is a derivation of the old English word 'Cade', meaning 'Of Obscure Origin'. It's usually applied to foundling animals.

My Great Grandparents were Russian, and I got my love of Russian language from the bits and pieces of conversation I had with them when I was a little kid. So, most times, I pick Russian names for my character's. Skyrim was no different. I named my High Elf mage Душа нокаутер, which means Soul puncher in English. Felt fitting for a magic user who could rip your soul out and stick it in a tiny little gem.

Default name if it's possible, my own name if I play Pokemon, if I make my character look like a character from a different game I name it after him. In Pokemon Soul Silver I named my rival Asswipe because it fit.

I tend to use my own name for male characters, or female names I really like. For example, my first FO3 playthrough was as a female character, so I used the name Alice, because it's one of my favourite female names. And in Halo Reach, my Spartan is female, so I gave her the service tag N147, because it's almost like 'NIAH' - very close to 'Niamh', another of my favourite girls names. Pronounced 'Neve', for the record. It's Irish, meaning 'bright' :D.

Either Andy, or something akin to Fagballs. Bulging Ballsack. Cuntface. Miss Tits. It really infuses every adventure with excitement and intrigue. Who is this mysterious Chesty LaRue? What is she doing here? Where did she come from? And why is she teabagging all the chickens?

I guess we'll never know.

I always go by Annoying Turd, or a variant like Annoying Loser. In multiplayer games, those names suit my playstyle well. I don't know how much weight such names carry in single-player games.

I'm boring, I just use my default intenet alias, Hawk, or some variation thereof, as the name for everything. Mainly because I'm terrible when it comes to naming characters, but also because a) they never mention the name in game in any meaningful way, so why bother, and b) the only time you ever really do see it is on the save file, so I use Hawk for my default, and maybe Hawk of Battle or BattleHawk or whatever for any alts. Makes it easier to find which is which.

Generally something that "shows" really sharp ice or snow. For Skyrim my main is called Icicle.
Also that bit you mentioned was spoiled for me because some idiot that joined the party decided to be a dick.

I tend to use some variation of my own name, depending on the context. Either I'll use the name I tend to go by, or I'll stick a "J" in the middle of it so it looks and sounds vaguely norse, or I'll just spell my second name backwards.

All of them work.

If your name isn't Senor Vorpal Kickasso, how do you expect to get anywhere in life?? All jokes aside, I usually use the name Max (Surname: Hunter, if required), Or some variation of Max...but for Skyrim, Vorpal is Dragonborn!!

Jeff. My character's name is always Jeff or Jeff Lawrence. And if possible he will always look like Captain Price.

I have a few names I tend to cycle through

Females:

Bellephoebe (female scholor-mage, current Skyrim character name)

Lasade (evil warrior-mage types)

Paige (a fairly neutral name I use)

Ethelberthina (goes by Thina, sometimes spelled out, usually female barbarians/rangers for the lulz)

Males:

Gilshalos (typically a mage)

Sholos (or Sholos the Smasher, most of my brute force warriors)

Elishium (polymorph characters, based loosely on the name of a prophet from The Bible, used mostly in the old Ultima games)

Grymauch (from a David Gemmel novel, I use it for scottish-themed fighters, barbarians, rangers, and similar)

Therumancer (my most frequently used mage-name, intended to be mysterious like the guy calling himself by a title rather than a birth-name)

Thorley (when I play dudes who are intended to be more refined fighters or socially oriented... swishy poke swashbuckers, rogues, etc...).

There are some others, I've been doing RPGs for a very long time, I tend to wind up with my characters being about 50-50 male or female either on computer or PnP. I probably have more names for dudes though because while it's been many years now I guess I used to play guys exclusively.

I always go with Legion, and before and ME fanboys start squeeling with glee, I was using it waaaaaaaaaaaaaay before that game. I actually started using it after watching "Storm of the Century", a fantastic movie in which an ancient evil, the Legion demons of Christian mythology, is basically reborn into a new form every few hundred or whatever years.
Actually on hearing the name I tend to think of the old Roger Zelazny book "My Name Is Legion" more than anything, especially when it comes to science fiction.

Honestly I sort of suspected that this might have influanced the name choice (the title) in Mass Effect given that it's a pseudo-classic by one of the masters of science fiction. You'd have to read it to understand, it's been a while and I don't want to give any spoilers. It's sort of like a science-fiction spy story with some odd conceptual twists.

Really it depends on the species of the character and the setting of the game. (medieval future ext) If its a medieval game and i'm an elf it would be something like Farathorn or if its a future/modern game and i'm a human it be a name that means something to me like James (seeing as its my middle name) either that or the default name example: my mass effect characters names are always either Jack (my best friend) or James and my grey warden elf in origins is farrow and the human is the default name Aiden.

When I'm given the option to name my character, I name them Pants. It's generally inconsequential anyway, but did lead to some fun dialogue options in New Vegas where they insert your name into some of the things you say, even though you never hear your character speak (and they wouldn't dynamically generate spoken names anyway).
