The WoW achievement Insane in the Membrane, it takes months of straight up grinding. And even then, you need either a rogue yourself or a rogue friend to farm thousands of lockboxes for the syndicate.

The individual instrument challenges in Guitar Hero: Metallica are infuriatingly difficult; the drum isn't that bad, and the guitar is do-able, but the bass is ridiculously difficult, and the vocal challenge is practically impossible because of the broken singing. I also find the achievements that require 4 people to be really stupid.

i actually got Little Rocket Man in the 360 version of The Orange Box for, wait for it, 30 points. I started out thinking it would be fun and easy, but keeping it in the car was legendarily annoying. I had invested so much time in the damn thing at that point that I felt like I couldn't quit it then.
An example of a good achievement can be found in Bioshock 2 in Ryan Amusements. Use telekinesis to smash a golf club into one of the wax Andrew Ryans.

Mass Effect 1's achievements actually boosted your character, which was nice. However, I'm questioning the person who had the idea to make the "Complete 80% of the game with this character" achievements. Tedium at its finest and the only thing forcing you to play the game over 3 times (doing ALL the side missions). It wouldn't be so bad if some of the planets weren't filled with so many mountains that you curse the planets' level designers, but there you have it.
in mx vs atv untamed there's an achievement for watching the credits to the end, and its worth 0g!!! wow don't i feel accomplished
There's a reward in the original Smash Bros. for beating Very Hard while taking NO DAMAGE.

Two words: Vidmaster Annual, mostly because every single one of my friends had done it already when I decided to go for it and no one was left. Then my one friend who promised to do it with me if I found other people went and did it behind my back. I still don't have that achievment then again I was never any good at Halo so it probaly would've been a challenge.
