Most of the time, I use Kain. Just my typical standard.

I use Torrigan for any elvish PCs,(cookie for reference).

This became a bit of a problem my first playthrough of Dragon Age.

There is always at least 1 Crim. After that, it's up in the air. I may use random, I may use a personality flaw (lol Laughy the Gnome Assassin).

I like to pretend I'm being witty with them.
If they come with a stock last name, I'll think up something that compliments that. If people say their name in text dialogue (like Pokemon or LoZ), I'll call them something that could make the game moderately more amusing.

Otherwise, I just give them something silly excellent and manly like Geronimo Montgomery Von Fabuloso or Killdark Demonfucker.

Mass Effect's been the only name-your-character series I've played (besides Final Fantasy but I like the canon names there) and I generally tried to keep the names as J-something:

Male Shep - Jack, John, Jim, Jacen, Jericho.

Female Shep - Jen, Jess, Jill, Jade, Jaina, Jewel.

There are others but I forgot what they are.

I think the 'worst' I have ever done is "Gaylick Shepard". The rest are names referring to inside jokes or just purely random.

I usually use Josef Stalin, or just Josef if it doesn't fit. If I replay the game I make up a name, I'm not very keen on using the default names because I like personalizing my character. My last Fallout 3 character was named Mr. Slippyfist.

I always come up with my own names on the fly. It's more fun that way. A couple which seem to always pop up for me are Lucretia, Cassandra (or some shortening), and Mongo.

My KotOR 2 playthrough involving Sith Lord Mongo was particularly excellent. "Life just pawn in game of Mongo..."

I tend to leave 'em be... or use a name from a novel I read recently if they're a little too bland or ridiculous.

I always name the main character my own name (at least the first time I play it), and I usually just keep the other's I'm able to name as their original names.

As do I. Unless there's a canon name (ie Link, Issac). DAO was the only one I got creative. And by "creative" I mean naming my PC after LOTR or Forgotten Realms characters.

If I don't use my own name, unless their name really suits them I'm likely to change it. My favourite thus far has been Madiken the Black Mage, from FF9. His original name was Vivi, and I still think mine was better. Daro the Hero of Time was alright too.

I used to use my own name all the time, because I really like my name, but after a while I noticed that I kept using it for characters that ended up doing silly things that I would never do, so I had to stop. It was a good call, though I still use it for Pokemon!
