Transformer Identification Help

Okay, this thread has nothing to do with "Transformers", it has to do with transformers as in a power converter. Now that thats out of the way, I am having issue with this transformer and I need to replace it, I know nothing about it other than it has 10 leads (4 of them larger than the other 6) and it has this gibberish on the top. Now, I am unable to identify the product number, make, etc etc from said gibberish.

Any help?

yeah that was really where i thought this was going to go as well... too bad really I had my lexicon of decepticons ready to go.

Sorry I can't help on the OP other then to suggest typing some of those identifying numbers into google with the word transformer in front of it.
