Urban legend games

Many, many long (and sleepless) nights have been wasted on reading about these games. Polybius. Killswitch. We know they don't exist and yet there's something fascinating about how gamers around the world dream them up and push the hoax forward. Religiously in more ways than one. How about we try to make a list as "complete" as possible? I'd love to see those games rounded up once and for all.

A few conditions though: no "haunted" or "hacked" versions of real games (i.e. creepypasta concerning Pokemon or Legend of Zelda), no fake postings either (i.e. don't just come up with a random creepy game; try to stick to an alleged source or web rumors rather than make up stuff on the go) and let's try be as informative as possible about dates, developers and misc.

I've made a thread about a game by the name of Imscared: A Pixelated Nightmare that seems to be based off of video game creepypasta: The game speaks to you, new files add themselves when you get caught by White Face at times, and there is one point where the game starts itself back up to scare you. It's by some guy named Ivan Zanotti, and there isn't a whole lot of info about it. You need to play/watch someone play the game yourself for it; I don't think there are any reviews and the only info on the download page is "I need a Heart in order to open it".

OK, the game does look creepy in a Slender kind of way but it seems to be an actual game? I was thinking more along the line of hoaxes like Polybius and stuff. I'd love to make a list.
