Have you ever been hapily playing a game, maybe a good 4 or 5 hours into it when suddenly. It dawns on you. Nothing happening around me makes any sense what so ever. Or a core usually unnoticed or just accepted mechanic/storyline suddenly seems to make 0 sense. The atmosphere and seriousness of the game breaks instantly and the experience is detracted from. It seems the designer didnt think at ALL about putting this into the game.

An example: Died for the 50th time on dead space 2 (zealot). I realised that EVERY time i died, one or all of my limbs were removed by the enemy. The whole purpose of necromorphs is to be created from whole corpses with all their limbs so they can stay intact. However they seem to not only be BAD at making WHOLE corpses without dismembering them, it seems it is IMPOSSIBLE for them to kill someone WITHOUT making their corpse useless for making into a necromorph. Although small it struck me as very stupid, and ive been unable to take the game seriously ever since. The whole necromorph thing makes absolutely no sense at all.

What other games have these usually accepted gameplay/story elements that tend to fall flat at the tiniest observation?

Well I mean, most of the Dead Space monsters are built on the idea that they are made from the scraps of people, hence why most of them are so horribly disfigured.

Also think about every game with a HUD despite the player character not wearing any kind of helmet/device for tracking such data.

I thought one of the cutscenes had someone turning in to a necromorph and they were sprouting new limbs. Although I may be thinking of something else. Or confusing the regenerator with everything.

Why/how does anyone die in games with raise-dead magic, or acknowledged respawn points?

I thought only those flying fucks made the new necropmorphs? It's been a minute since I've played, so I could certainly be wrong.

OT: I always thought it was odd that things like Pokemon or Final Fantasy Monsters never group up to just dominate humanity. Many of them clearly have intelligence equal to or greater than ours, so what's stopping them?

In Dragon Age, why do people stare at you and still talk to you when you have blood splattered everywhere on yourself? Yet, they talk to you abut the weather and such :/
