The Lippians from the Legaia games look pretty bizarre.

Though you'll probably find a lot more disturbing stuff in Shadow Hearts and Shadow Hearts Covenant.

The Monster Hunter series had a few bizzare looking monsters like e.g. Gobal is puffing fish with frog like legs and a angler antenna
Or how about the Qurupeco?

Also in Phantasy Star Online the final boss Dark Falz is a complete WTF of a flesh mess creature thing!

Monster Hunter creatures, not so strange to me.
Neither the Lippians creature. It is just a plant with a mouth.
Also, I already have Shadow Heart in the list.
