Black, by Pearl Jam. That song gets me. I couldn't listen to it without tearing up for about the first 20 times I heard it.

Runner up goes to Johnny Cash's cover of Hurt. Props to Trent Reznor for getting a legend to cover his song, but Cash did it better.
