There's a really good book on the subject called Female Perversions (there was a narrative movie made using the case studies from it but it wont make much sense unless you read the book first) which delves into many different fetishes, how they're exercised, and how they become imprinted. Not all of it is spot on, I for example don't relate at all to the experiences of others who share my fetishes.
Then again I don't have a hardcore fetish (where I need the fetish to achieve orgasm) and I think that's what the book is mostly about.
