Games sacrificing gameplay for style or plotline. I didn't even CONSIDER purchasing Kane and Lynch: Dog Days once I saw gameplay footage featuring the "shaky cam" style.
Sloppy/imprecise controls. Few things tick me off like getting killed because I was trying to jump over a chest high wall and the game thought I wanted my character to have marital relations with the wall instead.
Sacrificing/shortchanging single-player to focus on multiplayer.
Cutscenes where a bad guy shows up and does something while you're forced to stand there like an idiot and not shoot/stab/punch him in the face. I make an exception if there's a REASON why you're unable to act (stunned by an explosion, paralyzed by a spell, you're under a flag of truce, etc). This goes double when one of your teammates gets killed while you're sitting on your thumb.
Bad checkpoint/save systems.
Enemies that can kill you by repeatedly stun-locking you.

Oddly enough, I had no problem with the Virmire dilemma in Mass Effect
