A couple of times. Usually though, the game mechanics stop you from doing what you'd normally choose to do in that situation. An example would be most first person shooters, or 3rd person shooters come to that. I'd normally go around through the terrain and pick off the enemy stealthily, but the game itself doesn't allow you to do that (if it did, there wouldn't be a Halo 3 because Truth would already be dead by my own hands :D). Otherwise, can't say I've been in that situation...

One thing I hate is the common phenomenon for the protagonist to constantly carry the idiot ball [http://tvtropes.org/pmwiki/pmwiki.php/Main/IdiotBall] and never ever get anything that is blatantly obvious to anyone with two working braincells. And when they actually do get it, they have to speak it out loud to the entire audience(the player) what the big secret was, even though most people figured it our by the time we received the first or second hint.
Although this is most likely a case of All Viewers Are Morons [http://tvtropes.org/pmwiki/pmwiki.php/Main/ViewersAreMorons].

Every JRPG Ever: Shoot the main villain in the back when you first meet them and they temporarily join your party to showcase how awesome they are before betraying you.

I'm looking at you, Sephiroth/Ghaleon...

I never immediately thought of it while playing Fallout 3 until someone else here mentioned it but as we all know:

when you get to the point at the end of the game where you either have to end your own life or that of someone else by entering a radiation filled area, and considering you may well have a friendly supermutant, who is completely impervious to radiation, along with you. Why couldn't you just send in the supermutant to do the job for you?

That's one question I would really like to put to the creators.
