Sonic Adventure 1 (1998, on the Dreamcast). It's the one I play most consistently at least. Not counting stuff like Tetris or Pac-Man.

I still play Ultima VII (Both part I, the Black Gate, and II, the Serpent Isle), ~1994, as well as Ultima Online, ~1997

I also play the games from the Elite series (space sims), and the classic Elderscrolls games (Arena and Daggerfall).

Baldurs Gate 2: The Shadows of Amn still takes up way too much time of my day.

Mega Man 2, played so much I've memorised the code to get straight to the Dr. Wily stage.
A5, B2, B4, C1, C3, C5, D4, D5, E2. Urge to play rising.

I sometimes still pop in Breakout for my old Atari 2600.

And for some of the more new games I play. I still enjoy popping in the Adventures of Lolo games to my old NES.

I honestly play more old games than new games in general. Though those are the oldest games I had from my childhood that I still play. I spend more time ordering old games and importing games than playing new ones. It's often cheaper and it feels like a treasure to get old stuff, indie stuff, and imported stuff. New games can always wait. That's who I am, a weeaboo, retro gamer, import gamer, indie gamer, and yaoi fangirl.

The game system I play most often is undoubtedly the Super Famicom. It's not as retro as the original Famicom, NES, Atari 2600 or ect. But the system and games I play the most of aren't exactly XBOX360 or PlayStation 3 games. Even though I own both, and an enthusiast gaming computer with a 2500K and HD 6950.

There's just nothing like breaking out the Super Famicom and playing some games to study Japanese. I have an old SD TV in one of the guest rooms of my house just dedicated to retro games and old PC parts that I hook my old A/V systems into.

Do remakes and ports count?

If so, it's Super Mario Bros.

If not, it's the original Sonic the Hedgehog

Where in the World is Carmen Sandiego (1985, Apple II) It's way too easy now, but it's such a nostalgia trip that when I saw it on GoG (or somewhere) Windows 7 compatible and all, I had to have it!! I still have the original around somewhere, but not the Apple II to play it on!
