"Combat Veteran" as part of the Gun Runner's Arsenal pack in Fallout New Vegas.
I shot Benny with his own gun, threw spears at Caesar's head to cripple him and make "the god-king bleed", then I ran up and gutted Caesar with a Combat Knife.

I was going for 3 achievements in Deus Ex: Human Revolution. These were Legend (complete game on hardest difficulty), Pacifist (don't kill anyone but the bosses) and Foxiest of the Hounds (don't set off any alarms). During the game I got a glitch where my paralized enemies suddenly turned out to be dead, even though at first their status said they are sleeping. I also managed to save Malik which was easier than it seemed first, and at that point I was sure that at least one of the achievements I'll not get, but I tried to get them to the end anyways. Yesterday I finished the game and got all 3. Was the most rewarding feeling since I maxed out NFS Most Wanted. Now if there weren't any collectables in the game, I'd probably go for a full score but I don't care as there are collectables..
Probably finish all of Beast Mode in Gears of War 3.

Pro tip: An easy way to complete the weapon Vigils is to upgrade the weapon a couple times and do the proving grounds for said weapon.

I gotta say 'Craftsman' in Rise of Nightmares. I know it's a terrible game but it's fun so shut up.
Basically, you had to cut off three body parts on a creature leaving it alive in the process. It's much harder than it sounds but oh so satisfying when you get it.
