I guess it really depends on the type of game. If it's more modern, I'll make it up as I go, otherwise I use one of my many stock names.

From Levafae to Shyr, whatever floats my particularly proverbial boat.

My gamertag is DeltaSpartan5, if that helps.

I have four D&D characters that I am using:
1. Vargach Caex, Male Dragonborn Favored Soul of Bahamut - His name translates to Battle Sword
2. Xessia Drathuli, Female Half-Drow Ranger
3. Arista Corithan, Female Assassin
4. Vorel Kothar, Female Dragonborn Shadowbane Inquisitor - Her name means Beautiful Demon.

I made up this one for Lord of the Rings Online but sadly it wasn't allowed, Tea Baggins, you see Baggins is the last name of Frodo and Teabagging is.. yeaah, I'm going to leave that up one to google.

For actual roleplaying I use things like Wolfgang, Azuron and Bellamy

Well, most of the Bioware games I've played lately have seen the rise of Gunther. Gunther (Shepard, Cousland, Hawke and what have you) looks like a washed-out German ex-pornstar with a moustache to match. He doesn't take shit, and mostly doesn't give one either. His armor in Mass Effect 2 was pink, and he liked to get way too much into people's personal space, often with a shotgun.

Basically, Gunther came to be when I squinted at the default Mass Effect male face hard enough and summarily executed it with random slider-waggling. AND HE WAS A BEAUTIFUL, STUMPY BEAST! He looked like you or me, not the perfect, sculpted monstrosity they tried to saddle me with. From that horrible thing's remains, a proper man was born.

I've also got many tabletop character names, but they tend to be more or less common and not all that intresting. Although I like Empalsus Multar a whole lot.

You can find me anywhere under the name 'Illesdan', I don't like going under multiple tags. I used to use 'Tiger White' when I was writing fan fiction damn near twenty years ago, but I haven't used it since I turned eighteen. I have a D&D (pen/paper) lvl 16 half-elven ranger named Dirona Blackwater. When I first started running amok on the internet, I used to use that name, but for some reason, I got really sick of it really quick.

For fantasy games, I usually try to be original and come up with a name myself, usually starting with 'J', like J'Dhann

now and then I'll get lazy and borrow something from Norse mythology, like one of Odin's or Thor's alternate names

Most other games, I go for something simple, like Greed or Scar

Depending on the setting and my intended karma, I go with Sir Prize or Knight Maer. If it is in asia, I go with Wang Pu.

My tabletop characters all have their own names, but when I used to LARP I went by Elwyn. My character was a quarter werewolf who was kind of ostracized from his home, so Elwyn was kind of a bastardization of the initials "L.W." which stood for either "Lone Wolf" or "Lost Wanderer." I tend to adopt his personality, accent, and mannerisms whenever I do any sort of live roleplaying, including ren faires.

My first name when it's just a 1 player game. I dont' bother giving it a name. Craig 1 Craig 2 Craig 3. That's how I roll and have since Legend of Zelda.
