Age of Empires and Sim City 2000:)
Age of Empires here as well. I played the original Sim City a bit, but i couldn't get the hang of it. Probably because I was too young to understand that loans had to be paid back.

Earliest back I remember:

T'was a Gameboy Color, and two games: The Legend of Zelda Oracle of Ages, and Pokemon Silver.

It took two months to figure out how to save. I was a retarded child.

Warcraft III when I was a young-un. As a first or second Mature game (Both, I'm positive, by Blizzerd) it was fun. Sat for days on single battles just playing, save, and come back. Good times.

I got addicted to playing games when I was four-ish. My uncle told me about this game called Mortal Kombat II and I loved it. Then I moved onto the Donkey Kong Country series and so on and so forth. My addiction peaked in middle school, but kinda petered off during my high school days. Now I just play games here and there whenever I get bored or see a game I think is interesting.

I'm pretty sure I had a clinical video game addiction to Runescape for upwards of two years. If my highschool grades are anything to go by, at least.

You know it's getting bad when you spend all day thinking about a game, waiting for when you can play it again when you get home.

You know it's getting worse when you realize you're staying up until 4am playing a game on school nights.

Sonic the Hedgehog (the original) on the Master System, sometime in the early '90s. And this makes me feel old reading some of the other responses on this thread.

Edit for clarification: I'm not sure if the very first Sonic game was the Master System or Mega Drive/Genesis version, but I'm referring to the Master System game sold simply as Sonic the Hedgehog rather than as Sonic something-or-other.

For me, it was WCW vs. nWo Nitro on the N64. What great days I spent augmentating the looks of Hulk Hogan and Kevin Nash. I was but a tot, unwary of the other games such as Quake and Half Life.

i loved playing Wrestlemania 2000, that doesn't connect much to your post but it's a wrestling game even if you didn't have Hulk Hogan unless you made him, that kept me going for years, in fact i still got it...

When I was still in dipers I would watch my dad play a game called Ularn (which is "Ultimate Larn" for you not familiar), and as soon as I was able to understand how to play, I was hooked on pretty much any Rogue based game.

Rogue, Larn, Angband, netHack, et.al. were my first video game addictions. Longest lasting too, I still play them from time to time.
