I do know one thing for certain... apparently Ikaruga is classed as the world's 3rd hardest game. I recommend to everyone with either a Dreamcast, Gamecube, or Xbox 360 to try it. It's hard but fun.

I have to say Ninja Gaiden for the Xbox, real men played it regular, and not Black.

Seconded. I Wanna Be The Guy wasn't as hard as people made it out to be, I have beaten it, and many people have beaten it, so it is definitely not the hardest game. My friend who has that Ninja Gaiden was always complaining to me about how incredibly difficult it was, so I have to agree on that point too.

Ninja Gaiden games are freakin insane.

GTA:San Andreas and their flying segment. The game just comes to a complete halt and I haven't been able to beat it at all. It's hard enough flying normally with their sloppy flying controls, but when they make entire storyline-missions based off of it, I find myself completely incapable.

If you had asked "What's the hardest game to beat" I would answer Pac-Man because there is no ending, in the original version. But the hardest game would be Battletoads.

Actually, it is generally accepted that you 'beat' Pac-Man when you reach the level where the level counter overflows (aka the kill screen). This has been done by several people. But if a game has no defined ending I'd say that you can't count it when you're talking about games that are hard to beat.

For me (i.e. from the games I've played) it's probably Ikaruga, or IWBTG on hard. There was also this one version of Tetris where they put in an algorithm to always give you the worst block for the current situation...
