I would like to nominate Decado the Ice Killer from David Gemmell's writings.
Not only is he recognized as his world's penultimate swordsman, but he literally feels no fear (an enemy attempted to use magic to make him afraid, couldn't do it). Oh and his crowning accomplishment was dueling an enemy who was reading his mind and winning because Decado was just so damned fast that it didn't help (a very cool scene, his opponent sees the blow that kills him a quarter of a second before it does).
Well, that's just another reason why Samurai Jack's the best swordsman...

The greatest swordsman is, of course, Nightmare of Soul Calibur 3 and up. Why? Because none of these other yokels is the sword, but he IS Soul Edge and therefore 100% in control of his sword skills, having not to learn anything about wielding himself at all.
I have to agree with you thiosk. Commissar Cain is absolutely the best swordsman. Maybe not the most deadly, but definitely the best.
Years ago, back when I was part of the Arraxis Prime PDF I had the privilege, no, The Blessing to see the great Commissar Cain in action. There we were, a lone squad of Arraxis' best, facing down a mob of chaos cultists. Our ammo was low, and our moral was even lower.

Nah, Worf never actually fought Kahless directly, classic or clone. It was Gow'ron, the then leader of the High Council, who defeated the clone in single combat... mostly because that Kahless was more interested in raising the spirits of the Klingons and "reaching" Gow'ron philosophically than actually defeating him. Of course, that's no excuse. Losing due to lack of focus is still losing.

It HAS to be Inigo Montoya. Was a child-progeny at age 11, spent 20 years studying swordplay and could defeat the Grand Master Swordsman of the world (they don't mention most of this in the movie, but there is a 25 page chapter devoted to him in the book).

Erza Scarlet from Fairy Tail would probably be very high up.
Why:
She does use magic to equip armor/weapons, and she does get magical enhancements from said equipment. But even without those she's an absolute beast. Not just in strength. She's very fast, her stamina seems to never fully run out, and she's able to assert an enemie's strength and weakness by just attacking them once. Sometimes even by just observing them for a few seconds. On top of that she's an expert with all kinds of weapons from hallberts to spears to swords to daggers. And she can dual wield just as well too. In a world full of magic, she only needs a sword to beat all but the top ranked mages. If she uses her magic to equip different armors and weapons with different benefits, only guild masters or people of similar strength can still beat her.
While one might assume that Yammamoto's sword skills are superior to Kenpachi's, we can't know for certain.
We do know that Yamamoto was more powerful than Kenpachi in a general sense as he was able to "force" Kenpachi to take kendo lessons.
But speaking of Kendo, no character has been shown to receive the massive power boost Kenpachi receives when using Kendo. Could this be because he's because he's so skilled at it? After all he learned Kendo in only one lesson and was able to utilise it great effect in spite of not using it in a long time (which considering it's Kenpachi, is probably a really long time).

Kenpachi was able to beat the most powerful captain of her time (Retsu/Yachiru Unohana) when he was only a young boy. He only lost as he suffered a mental breakdown upon realising he could beat her and put mental blocks upon himself, limiting his power and skills to a tiny fraction of what they once were. In his current state in the manga he has had these blocks removed and is an exponentially more dangerous opponent because of it. While we have yet to see him fight in his new state it could be quite possible he is now the most best swordsman in bleach
If I'm being honest though. I wouldn't pitch Kenpachi as the best Bleach character for this thread. It's a question of which character would do the best in a gigai and I think that would be Unohana. Her zanpakuto is useless for fighting, she's never been shown to use kido offensively, yet she's known to be a master swordsman and is commonly thought to be the second most powerful captain. So it stands to reason that she has the greatest pure sword-skill out of all of them.

I like them both so don't ask me to pick.
I would also like to nominate Jubei from Blazblue mainly because in a world of of super-powered beings nobody wants to mess with the dude. The problem is that what he can do and if he does actually qualify is up in the air because he has yet to display his skill in game.
Special mention goes to Citan Uzuki mainly because what makes him badass is his plethora of skills, and not just his skills with a sword.
Hear me out here- This is more Christopher Paolini being silly than him being a badass. The kid manages to go from total novice to be able to match Brom (a formidable, ancient veteran of an order of the greatest magical warriors evaaaar, if ageing a bit) in a few weeks if I remember rightly. I'm pretty bloody sure that's ridiculous. Later on he gets some funky dragon power up thing that effectively makes him an Elf, so no one bar two or three people in his universe can hold a candle to him (I never got around to reading the fourth book, so I don't know how silly this all gets by the end).
Now, hypothetically Eragon is immortal and ageless, baring any injury. He has infinite time to hone his skills with a body that will not only never wither, but is also superhuman. That's just OP.
Jaime Lannister (A Song of Ice and Fire/Game of Thrones) was widely considered to be the best swordsman in the Seven Kingdoms, and possibly the world. Don't know if he could actually compete with some of these people, but the dude was a badass.
