You'll probably understand it on the 'Shoot things to survive' level of understanding.
But as far as story and characters go... no, I don't think you would if you know none of the backstory. You may be able to piece together some of it, and I don't know if you can read the backstory of previous games like in the other games.

I wouldn't hurt to read the plots of the old ones, this way you can get a understanding of the characters.

OT: You need to start at the first really, that games plot is so muddles that starting at the end just doesn't work.

I got 4 before I got 1,2 and 3 and I understood the story mostly... it will still be good. But it is highly recommended you play the others before.

Even if you don't play the previous MGS games to get the story/plot you can still play MGS4 and enjoy it as it really is a fantastic game :)

Start from the beginning of the series. There's so much info, that, it can be a bit overwhelming.

OT: I played MGS4 before any of the others and I was completely fucking lost even if I had a map.
