Worst Glitch You've Ever Experienced?

So I've been playing Saints Row the Third for a few days now and so far I've come across some pretty annoying glitches; namely that my car occasionally explodes for no reason and occasional black screen freezes.

Other than that it's a good game, but it got me thinking: What are the absolute worst glitches you've ever come across in a game?

The worst one I've experienced was in Darksiders, wherein the final dungeon had a particular elevator that you could fall through the bottom of as it was going up and it would never come back down. This was either annoying if you missed a collectable or game-breaking if you hadn't fought the sub-boss up there.

In fact, ALL the worst glitches I've experienced involve falling through the map.....hmm.

I guess I'm cursed.
Maybe you never turned off the noclip in Doom 2 and it's followed you ever since like Ring.

I've been pretty lucky glitch wise as I've never encountered too many of them even in known glitchfests like Fallout:NV.However some of the more annoying ones I've come across are

Rush N' Attack:Ex Patriot - Right after taking an elevator up I died.The game respawned me back before the elevator.Unfortunately the elevator was still up and there was no way to make it come back down meaning progress was impossible and restarting the game was required

Fallout 3:The Pitt - Kept falling through a seemingly solid roof and dying.Meant collecting the 100 steel ingots was impossible

Fallout 3:Mothership Zeta - Right after the spacewalk bit you have to activate a teleporter and a little girl comes through and unlocks the door for you.Problem is she didn't meaning progress was impossible.Required reloading previous saves multiple times

Kotor 2: Got 95% through the game
the return to the Dantooine Enclave where Kreia kills the masters (lightside)
and couldn't progress past the load screen to leave the planet. Gamebreaking bug that forced me to start from scratch at the very end of the game... Kotor II is a long game.

Missingno ate my save file. Granted that was my own fault, but still. Other than that I've been pretty lucky as far as bugs go. The only recent entry was F:NV Old World Blues when I got stuck in a conversation loop and had to console my way out of it.

Fallout New Vegas but I'm not sure what was more annoying: the quests that got locked or the constant freezing.
