Worst time you ever mis-spoke

This happens to everyone at some point you know when you're pre-occupied with inner thoughts or whatever but basically when you say something you didn't mean to say by accident.

I have one incidents this is one I STILL hear about today even though it happened two years ago (now almost three)
Basically it was 2007 and my raunchy friend who was and still is into telling countless dick jokes had just gotten an X-box 360 he didn't have live yet but we played Halo 3 split-screen modes constantly, he of course told a dick joke everytime he killed me or I killed him, understand I was being buried under an avanlanche of fucking dick jokes at this particular moment. So halfway through the game he started jokingly using a Monty Python-like Britsh accent. I had seen videos on YouTube of people doing fake voices on XBL to mess with people.

So I meant to say something along the lines of "You can plug in the mike and do that voice of XBL..." thanks to those god damn dick jokes in rapid fire sucession I instead uttered the now infamous among my friends "You plug in the cock" and the rest was history the worst part is he has a really loud obnoxious laugh that makes it really hard to explain to him that you mis-spoke. I STILL hear about this today they're never going to let me live it down.

So my question to you is whats the worst (I believe they're called gafs or gaffs but not according to dictionary.com when I look up both words) mis-speaking you've ever expirienced this could be out of pre-occupation, impulse etc.

My brother was once in a play in grade school and during the play a girl had to refer to him as "Honey" and he was mortified when she did.

I still make fun of him for it.

Similar thing happened to me.

My friend was making a joke about my friends dick and instead of saying "Every body crouch" over the mic I said "Every Body CROTCH!" (A couple of times)

I have a lisp, a cracked tooth, a low attention to what I say, and occasionaly start talking like porky pig. There have been numerous times I have mispoken in quite terrible ways, especially since my family loves dirty jokes, things my grandmother can't seem to stop setting herself up for.

I do this often, like at least once a fortnight, hard to remember any single good one, but one recent one was:
We were watching UP, 1st time i'd seen it, well, most of us in fact, and i make the joke when there's a bunch of clouds looking like babies, saying '400 babies!' with a impression of the powerthirst guy, which was very funny until the very next bit, then it was a horrible, horrible, thing to say.
