I know you said no anime, but the worst I've ever been offended by a voice actor was the two main characters in the english dub of Revolutionary Girl Utena.
OT: I personally reckon that Resident Evil 5 has to take the cake for this one. The writing AND acting were so monumentally bad that it made you laugh your arse off.

Not sure what your standards are, but Dante, Vergil, and Lady all had very memorable parts in DMC3 that were a lot of fun. For DMC4 Dante sounded very cool as an older and more mature badass and Nero took over the young angry guy role competently. I just don't see the point in saying that ALL Capcom games have bad voice acting, that's not in the spirit of the discussion.
